360 feedback review forms are designed for peer-review scenarios.
Creating a new peer review request
To distribute a 360 form directly to individuals who are required to give their peer feedback go to:
Performance > 360 Feedback
Click the 'Add' button to add a new feedback request. You must first select who the feedback is requested and the recipient who is to give the feedback on this person. Note that only employees with a user account and a valid work email will appear on the recipient list.
Next, you can set an expiry date on the feedback and select whether you are using our standard 360 feedback questions or your own custom questions, from which you will need to select your 360 feedback form.
To create a 360 feedback review form, go to:
Administration > Company > Form Builder
Then, click 'Add' to create a new form. For further information on creating custom 360 forms, please see our article on Form Builder Form Types.
Please Note - When creating a 360 feedback form, you have the option to make feedback entries anonymous so the employee's name will not appear alongside any of the responses.
From the main screen, you will be able to see your current 360 feedback requests in the system. You can see who the feedback was requested when it was requested and whether it used a custom 360 feedback form or the system standard predefined questions.
You also have the option to send it out to additional people using the 'Add recipients' icon in the "Actions" column. Other options you have in the actions menu are to change the expiry date of the request, archive the request, and download the responses to a CSV file (custom form only).
Email Notifications
Using the standard 360 feedback form, the employees' manager will be notified when a requested user has provided feedback on their employee.
In the below 360 feedback example, the feedback has been requested on Angela Martin whose line manager is Mark Cuban. Another employee has been asked to give feedback on Angela - when they have done so, the line manager Mark receives the below email prompting him to log into the system and view the 360 feedback requests.
