Form Example: Monthly Review

This form recipe will detail the steps required to create a simple monthly review/121 form. If you'd like to use this recipe as a base-line for creating your own form it's a good way to get a complete form which you can add to or alter after it's creation. 

The Form

There are a few different fields within the form creation page which we need to configure, please find them listed below along with the values we'll use.

  1. Form Name - 121 Form
  2. Form Description - Add as required.
  3. Form Type - Performance Review.
  4. Visible to employee: No.
  5. Option to lock form response(s): Yes - Unlocked upon publishing.
  6. Table display date field: None.
  7. Table displays label field: None.
  8. Form Tag - Add as required.
  9. Rotate Download - No (this is entirely up to you regarding how you'd like form downloads to display in a CSV).
  10. Job Role: Add as required.
  11. Only usable in workflow?:
  12. Trigger Workflow on Submission - Blank
  13. Status - Active.

Once those fields are all filled in click Submit, you should then see a green success banner.

If you now click the Build Form button you'll be taken into the form builder where we can add in our fields.

Form Builder

Employee Section:

  1. Field Display Label - Employee Section Start.
  2. Mandatory Field - No.
  3. Field Type - Section - Start.
  4. Values - Blank.
  5. Field Access - Employee cannot view or update.
  6. No advanced options needed.

Team Member:

  1. Field Display Label - Team Member.
  2. Mandatory Field - Yes.
  3. Field Type - Assigned Employee.
  4. Field Access - Employee can update field.
  5. Click the Advanced Options toggle and locate the "Uneditable once completed" field - Yes.

Date:

  1. Field Display Label - Date.
  2. Mandatory Field - Yes.
  3. Field Type - Date Field.
  4. Field Access - Employee can update field.
  5. No advanced options needed.

How do you feel about work?:

  1. Field Display Label - How do you feel about work?.

  4. Field Access - Employee can update field.

That concludes the creation of our monthly review form, you can add or takeaway form fields as you see fit, this is just a starting point so that you can then create a more bespoke form yourself.
