Form Example: 9 Box Grid

This form recipe will detail the steps required to create a simple 9 box grid form. Completing this form will populate the employee's 9 box grid data within Performance -> 9 box grid. If you'd like to use this recipe as a base-line for creating your own form it's a good way to get a complete form which you can add to or alter after it's creation. 

Any of the 9 box grid specific fields will be marked as (standard) in this recipe but depending on your 9 box grid settings you may want to instead use (custom) fields instead.

The Form

There are a few different fields within the form creation page which we need to configure, please find them listed below along with the values we'll use.

  1. Form Name - 9 box (or anything that suits you).
  2. Form Description - Add as required.
  3. Form Type - Performance Review.
  4. Visible to employee: No.
  5. Option to lock form response(s): Yes - Unlocked upon publishing.
  6. Table display date field: None.
  7. Table displays label field: None.
  8. Form Tag - Add as required.
  9. Rotate Download - No (this is entirely up to you regarding how you'd like form downloads to display in a CSV).
  10. Job Role: Add as required or tick All.
  11. Only usable in workflow?: No.
  12. Trigger Workflow on Submission - Blank
  13. Status - Active.

Once those fields are all filled in click Submit, you should then see a green success banner.

If you now click the Build Form button you'll be taken into the form builder where we can add in our fields.

Form Builder

Employee Name:

  1. Field Display Label - Employee Name.
  2. Mandatory Field - Yes.
  3. Field Type - Assigned Employee.
  4. Field Access - HR only.
  5. Click the Advanced Options toggle and locate the "Uneditable once completed" field - Yes.

Image (as part of my form I've provided an image of a standard 9 box grid to assist managers/HR members while they complete the form):

  1. Field Display Label - Blank.
  2. Mandatory Field - No.
  3. Field Type - Image.
  4. Choose file to upload - (chosen image)
  5. Field Access - Blank

Performance Rating:

  1. Field Display Label - Performance Rating.
  2. Mandatory Field - Yes.
  3. Field Type - 9 box grid - performance (standard).
  4. Field Access - Employee Cannot view or update.

Potential Rating:

  1. Field Display Label - Potential Rating.
  2. Mandatory Field - Yes.
  3. Field Type - 9 box grid - potential (standard).
  4. Field Access - Employee Cannot view or update.

Risk of Leaving:

  1. Field Display Label - Risk of Leaving.
  2. Mandatory Field - Yes.
  3. Field Type - 9 box grid - risk of leaving.
  4. Field Access - Employee Cannot view or update.

Manager Comments:

  1. Field Display Label - Manager Comments.
  2. Mandatory Field - Yes.
  3. Field Type - Text Area - Multiple Line.
  4. Field Access - Employee cannot view or update field.

Completed

  1. Field Display Label - Completed.
  2. Mandatory Field - Yes.
  3. Field Type - Completed. (if you plan to use this form in a workflow use a 'Workflow Step Completed' field instead).
  4. Field Access - Employee cannot view or update field.

That concludes the creation of our 9 box grid form, you can add or takeaway form fields as you see fit, this is just a starting point so that you can then create a more bespoke form yourself.
