Form Warning

When building a form, there are a number of scenarios where a form may have issues that could impact the core usability of the form.

This is indicated by an icon next to the form status - the icons do not link to anywhere as it is only possible for this to be caused by a small number of issues. When you hover over the warning icon(s), the message will tell you the reason for the issue that will be one of the below.

The warnings can be coloured:

  • RED, meaning this is a major issue that can have a significant impact;
  • ORANGE, meaning it's a warning that may or not be significant.

it is possible to trigger a workflow from the form. Occasionally, the workflow which the form refers to may be deleted or inactive which will then cause unexpected behaviour when the form is submitted.

These form warnings are:

A red triangle indicates that it doesn't have an "Assigned employee" field. All forms of type Other, Performance review, and Performance improvement should have a field type of "Assigned employee" included. We strongly recommend that all forms of these types should include this field - otherwise, the response in the form will never be attributed to the relevant employee.
An orange circle indicates it doesn't have a "Completed" field. All forms of type Other, Performance review and Performance improvement should have at least one "completed" field as this controls the flow of the form from employee to manager within a workflow and can control who has access to the form.
A red person icon indicates that your form is not accessible by an employee. If forms of type Other, Performance review and Performance improvement is contained within a workflow and at least one of the workflow steps involves sending the form to an employee, the system will check to ensure that the form is accessible by the employee.
In other words, this means we will check the setting against the form to ensure that the "Visible to employee" option is set to 'Yes'. We will also check that at least one field in the form is visible to the employee.
A red manager icon indicates that your form is not accessible by a manager. If forms of type Other, Performance review and Performance improvement is contained within a workflow and at least one of those workflow steps involves sending the form to a manager, the system will check to ensure that the form is accessible by the manager.
In other words, this means we will check that at least one field in the form is visible to the manager.
This orange "code" icon indicates if you have an uneven number. If you have a form that has either sections, clones, or pages included, it is then important to ensure that these are all "closed" properly - that is for every start you have, you should have an end.
In this instance, we will check to ensure that you have the same number of starts and ends.
This orange "pages copy" icon indicates that if you have a form that has a clone section, you need to ensure that you also have clone fields in the field - likewise, if you have clone fields, you need to then ensure you have clone section.
