Self-Certification and Return to Work Forms

Within HRWize, it is possible to use the form builder to create customized self-certification and return to work forms which can be completed electronically following an instance of absence. 


What is the difference?

  • Self Certification forms require an employee to complete a section of the form themselves. Once completed, these forms are stored against the instance of absence ready for the employee's manager or HR to complete their section of the form. 
  • Return to work forms are sent directly to the manager following the instance of absence to be completed. The employee who the instance of absence relates to will not have the ability to complete or view this form. 

Enabling Self Certification and Return forms

Once the form has been created within the form builder, there are a couple of settings that will need to be changed in order for this to be enabled: 

First, select which Self-certification or Return to work form you would like to use by going to:

Administration > Company > Settings > Time off settings (global)

Under the Absence settings section, select the form you have created in either the "Self-certification form field" or "Return to work form" options.

Once the form is selected, click 'Submitat the bottom of the page to save the change.

If you require a Self-certification form or Return to work form to be triggered after a certain duration. For example, trigger a self-certification form if the absence is longer than 5 days, you should enter the trigger duration in the Return to work trigger duration or Self certification trigger duration fields respectively.

You will need to indicate to the HRWize system, which of your time off types requires a Return to work or Self certification form. To do this, go to

Administration > HR > Time off management > Time off types

Here, you can either add a new time off type, or edit an existing one. 

In the Absence settings section, select 'Yes' in the Return to work required or Self certification required options to indicate if this time off type requires either or both of these forms to be completed.

Once completed, select 'Submit' at the bottom of the page to save the change. 


Completing a Return to work form

Once set up, if a return to work form is required following an instance of absence, the employee's manager will receive a notification prompting them to complete the return to work form on their To Do List right hand side.

The manager will then be able to review the details of the absence whilst completing the form, by clicking on the item in their workflow.

Once completed, the Return to work form can be viewed at any time against the instance of absence under:

Time and attendance > Time off


Completing a Self Certification form

Once an instance of absence requiring a Self certification form has been entered into the system, the relevant employee will receive an email notification to suggest that this needs to be completed; the employee will also receive a banner which will allow them to complete the form on their To Do List right hand side.

Once submitted, this form will be fed back to the employee's manager to review and complete their section if necessary.
